Appointment Cancellation Policy

Whenever you need to cancel an appointment, we request that you call the above number at your earliest convenience. The freed up slot on the schedule allows us to serve the needs of patients who are on the daily waiting list to see Dr. Mann. We also want you to know of the following cancellation policies and fees:
At least 48-hour notice cancellation: No Charge
Missed appointments (No Show): Full Appointment Fee

Payment Information

Payment is at the time of service.

We accept cash or check.

We provide a superbill for you to remit to the insurance company for reimbursement if you wish.

Payment options are available if necessary.
